Weekly Report
Once a week, for the first three weeks of the chart, students will create a bottom line for the week and receive a weekly report of their progress thus far. This provides the student with a current trend in status, a comment on performance by the SHAPE teacher, an unofficial status of the program based solely on point slips, and is read in group as a way to determine a bottom line goal.
At the completion of the fourth week, a monthly chart is handed out to the students.

Monthly Report
Students will receive a monthly report every 20 school days. This report based solely on point slips and chart is a major report. It is an official report which provides a status in each class. It goes to the student, the student's parents, and the student's teachers. It is also used as a partial determination of the SHAPE grade on the student's report card.
The report is determined solely by the student in that it is tabulated based on the student's color coded chart. Thus, the final 20 day status received can be controlled by you by controlling what goes on your chart each day. The whole system is designed to help students make adjustments in areas where they need to improve.

Status Determination
Status is determined at the end of each 20 day assessment chart. Status in SHAPE is not something that just happens to you. Status is determined solely by what you have chosen to put on your 20 day chart.
Through your daily point slips and your weekly reports, coupled with your bottom line each week, it is your job to begin to find success in school by controlling what goes on your chart.
There are 5 Levels of status in SHAPE:
LEVEL 5 indicates excellence in school
LEVEL 4 indicates you are doing well in school
LEVEL 3 indicates at least one area where improvement is needed
LEVEL 2 indicates poor performance in school
LEVEL 1 indicates very critically poor performance
Level 5 is difficult to achieve. It indicates a student is performing well in all areas. In a negative way, Level 1 is also very difficult to achieve. It is cause for great concern. Parent contact with the SHAPE teacher is essential for a Level 1. It is also highly recommended for a Level 2.
Occasionally, a status level in a particular class may not be an accurate indicator of academic grade. At times, a student may be on time, doing homework, and behaving in class, yet still doing poorly academically. This is cause for concern. I would strongly encourage parent contact with individual teachers in these instances. An administrative parental meeting may also be a good idea.
